What Is Diabetes Education?

The diabetes education program at Superior is a course in self-management of a chronic, life threatening ailment. Diabetes is an extremely common ailment which can lead to a variety of serious medical complications. Further, these complications - along with diabetes itself - can reduce the quality of life for the individual and can cause premature death or disability. The good news, however, is that diabetes can be effectively managed, and with effective management most of the time serious complications can be avoided!

Diabetes management begins with a trip to your doctor where diabetes is diagnosed. Diabetes education begins on the way home where diabetes is managed. From the beginning, our program will work with your doctor to get insulin and other medications properly adjusted. Beyond proper medication levels, our program will provide tools to you or your loved one which will help you better work with your doctor to manage this disease.

Our program provides a certified diabetic instructor to assess you or your loved one. The instructor will help identify specific circumstances and lifestyle choices that may undermine effective management of the disease. Specifically, our program provides for the following services:

  • A good overview of diabetes and how it affects the body,

  • Design of an individualized plan of care than manages the illness through diet, exercise, self-monitoring and medication,

  • Individualized recommendations for exercise and for maintenance of a healthy lifestyle,

  • Instruction on blood sugar monitoring for better control of the disease,

  • Guidance in minimizing and avoiding diabetes-related complications,

  • Personalized meal plans which still include your favorite foods,

  • Guidelines regarding healthy food choices when eating out or “on the go,”

  • Help with motivation and the desire to make lifestyle changes,

  • Explanation of factors that affect blood sugar levels, and

  • Instruction in how to take care of yourself when you become ill.

Is Diabetes Education Right for Me or My Loved One?

If you or your loved one has diabetes, education, motivation and effective self-monitoring are essential to control of the disease. Our program can help provide the necessary tools to manage diabetes. Further, like other home care services diabetes education is covered by most insurance plans and Medicare.

How Do I (or My Loved One) Qualify for Diabetes Education?

If you or your loved one has diabetes, is currently homebound, and needs effective diabetes care, we can help. Under most insurance plans and Medicare, diabetes education is recognized as a skilled medical need that qualifies for home care. This need may initially be established by a doctor, but may also be determined by our staff during a home consultation.

Once you or your loved one has been certified (qualified) for diabetes care, we can continue to provide that care - along with other needed home care services - for as long as a skilled medical need remains. This need for skilled educational services will be reevaluated periodically during the course of treatment, and requires that the patient continues to comply with the educational program. When the patient's medical condition improves - or he or she ceases to comply with the program - our staff will work with you prior to discharge to insure that you and your loved one continue to receive necessary care giver services even when nursing or therapy is no longer required. In every case, we will work with your doctor and make all the necessary calls in order to care for your needs in a timely manner.
