Home Health Care Assessment

Superior Home Care & Hospice provides a free no obligation, in-home health care nursing assessment to people living in our service area who are likely to meet criteria for home health care services. If you or your loved one have great difficulty leaving home due to illness or injury, or if you have a medical condition that restricts your ability to leave home - except with the aid of supportive devices such as crutches, canes, wheelchairs, walkers, special transportation or the assistance of another person - then you may be eligible for home care services. Further, in many cases Medicare, Medicaidor private insurance will pay 100% of the expense.

The in-home assessment is performed by a Superior Home Care & Hospice registered nurse. A family caregiver may find it helpful to be present at the assessment, though this is not required. In most cases this assessment is provided in the patient's home. It can also be done in a medical facility at the time home care is first considered.

The assessment takes approximately one to two hours and may include:

  • Vital sign check, including blood pressure, heart rate, respiration and weight

  • Pain assessment

  • Medication review

  • Physical assessment, including ears, nose, mouth, skin, mental, neurological, muscular, cardiovascular and respiratory status

  • General health screening

  • Diet and nutrition assessment

  • Self-care ability evaluation

When the assessment is concluded, the nurse will offer a summary of your home care needs. This may include recommendations for community services, suggestions for lifestyle changes, issues to discuss with your physician and a description Superior home cares services that can help you live better now. If the nurse finds that you or your loved one are eligible for home care services, they can begin the process of admission on the spot.

Once admitted, Superior will contact your physician to obtain an order for future home health care visits. If you do not meet the Medicare criteria for home health care, we will refer you to other services based on need.
